Public register of verified robot and autonomous-vehicle incidents, with case timelines, company breakdowns and NHTSA crash filings.
What you can do
Search verified incidents involving commercial robots and autonomous vehicles, and open one case by reference or slug with its timeline, sources and linked NHTSA report
Browse companies with published cases, their harm and escalation counts, incident-type breakdown and category averages
Read register-wide totals: case counts, regulator or court escalations, archived sources, countries and last update
Query NHTSA Standing General Order 2021-01 crash reports by report or by incident
When to use it
Useful when you need sourced, dated facts about robot or autonomous-vehicle incidents, company-level incident histories, or the underlying NHTSA automated-driving crash filings, rather than general web search.
Requirements
Remote streamable HTTP endpoint at themachinerecord.com; no authentication, environment variables or headers declared.
Before you install
Read-only lookup service, but the register counts reported incidents and is explicitly not a safety ranking, so figures should not be read as rates or comparisons of machine safety. Case metadata is licensed CC BY 4.0 and the server asks that cases be cited by their permanent URL.
Enable the server for the chats that should use its tools.
Web executable via Streamable HTTP. Remote servers run from the web runtime once configured in a workspace.
Use with an agent
Paste this prompt into Claude Code, Codex, Cursor or another agent — or into SourceWeft chat. The agent reads this server's install guide, shows you its publisher, endpoint and sign-in, and adds it to your client once you agree. Keys never go into a command.
Read https://sourceweft.com/mcp/com.themachinerecord%2Fregister/install.md and connect the MCP server it describes to my client. Before changing any config, show me its publisher, endpoint and sign-in method and wait for my OK. Never put an API key in a command or in a file I would commit; ask me where it should come from.
Search verified incidents involving commercial robots and autonomous vehicles (robotaxis, delivery robots, humanoids, industrial arms, surgical and consumer robots). Every case has at least two independent sources and is followed from first report to investigation, recall, fine, lawsuit or settlement. Returns case summaries with permanent URLs; use get_case for the timeline and sources.
Full record of one case by its reference (TMR-0123) or URL slug: summary, machine, dated timeline of what each source established, the sources with their tier and archive copy, and any linked NHTSA Standing General Order report.

list_companies
Companies in the register
Every company with at least one published case: number of cases, how many went beyond the first report, and the share. Not a safety ranking: the register counts reported incidents, not incidents per machine.
Takes no parameters.
get_company
Company profile
One company's cases with harm and escalation counts, breakdown by incident type, and the category average for comparison.
Parametersslugrequired
register_totals
Register totals
Headline numbers: cases, cases that reached a regulator, recall or court, archived sources, companies, earliest year, last update, cases by country, and totals of the NHTSA Standing General Order file.
Takes no parameters.
search_nhtsa_sgo
Search NHTSA SGO crash reports
Crash reports filed under NHTSA's Standing General Order 2021-01 by automated driving system (ADS) and Level 2 driver assistance operators, 2021 to today, one row per report at its latest version, or one row per incident when grain is 'incident' (filings by several entities about one crash collapsed at the highest severity). Rows are the operators' own filings, not verified cases, and a filing does not establish fault.
How complete this listing is, computed from the registry entry, the endpoint and public sources. Not a rating of the server and not a statement that it is safe or working today: reachability is a dated check, and no installs or reviews go into the score.
