Requirements
Remote streamable HTTP endpoint; no local install. Sign-in is via OAuth, and on first connect you supply a Formitize API token (created in Formitize under Settings, API, API Tokens, with forms read permission), which the server stores per user. No keys go in the client config. Free plan allows 50 tool calls per month.
Before you install
Two tools write to Formitize: creating and updating CRM tasks, so confirm before running them. The server stores your Formitize API token per user. Paid plans cost money (Pro $9/month or $90/year, Pro+ $39/month or $390/year) and upgrades return a Stripe Checkout link; plan changes charge the price difference immediately and cancellations give no refund for the current period. Usage is metered per call.

Connect

  • Claude (claude.ai, Desktop): open Customize → Connectors, click + Add → Add custom connector, and paste https://formitize.usefulapi.io/mcp.

Add only the URL. Do not add an Authorization header or an API key to the client config: the server signs you in with OAuth, and the login page asks for your Formitize credentials.

Each paid plan covers this server only. Ask your AI to run formitize_upgrade and choose Pro or Pro+, billed monthly or yearly (it returns a Stripe Checkout link). If you already have Pro, ask to change it to Pro+. The change requires your confirmation, keeps your billing interval and charges the price difference for the rest of the period immediately. Cancel any time with formitize_cancel_subscription: your paid plan continues to the end of the paid period, with no refund for the current period, and asking to resume the same plan with formitize_upgrade before then undoes the cancel.
