Overview

AI-generated overview

Read-only remote MCP server that helps an assistant decode medical EOB and bill codes, insurance terms and printed amounts.

What you can do
  • Look up ICD-10-CM, HCPCS Level II and modifier codes for official descriptors and current validity
  • Search codes by everyday wording or code prefix
  • Explain insurance and billing terms with examples and related terms
  • Recompute EOB line or claim totals and flag balance billing or mismatched amounts
  • Return step-by-step guides for reading an EOB, disputing a bill or appealing a denial
When to use it
Useful when a user wants to understand an Explanation of Benefits, Medicare Summary Notice or itemized medical bill: what a code means, what an insurance word means, whether the amounts add up, and what to do next.
Requirements
Remote Streamable HTTP endpoint; no authentication, accounts or API keys declared. Works with any MCP client that supports HTTP transport.
Before you install
Read-only reference, not medical, legal or financial advice; responses carry a disclaimer. Inputs are limited to codes, terms and dollar amounts, and the server states it has no storage and does not log tool arguments. CPT, CDT and CARC/RARC descriptors are licensed and not included.

Installation

In SourceWeft

  1. Open KeepMD MCP for EOB Decoder in the dashboard and add it to a workspace.
  2. Enable the server for the chats that should use its tools.

Web executable via Streamable HTTP. Remote servers run from the web runtime once configured in a workspace.

Use with an agent

Paste this prompt into Claude Code, Codex, Cursor or another agent — or into SourceWeft chat. The agent reads this server's install guide, shows you its publisher, endpoint and sign-in, and adds it to your client once you agree. Keys never go into a command.

Read https://sourceweft.com/mcp/org.keepmd%2Fmcp-eob-decoder/install.md and connect the MCP server it describes to my client. Before changing any config, show me its publisher, endpoint and sign-in method and wait for my OK. Never put an API key in a command or in a file I would commit; ask me where it should come from.

Read the install guide the agent follows

README

KeepMD MCP for EOB Decoder

A read-only Model Context Protocol server that helps an AI assistant read a medical Explanation of Benefits (EOB), Medicare Summary Notice or itemized bill. It is the same job the KeepMD EOB Decoder browser extensions do, exposed to Claude, ChatGPT, Cursor and any other MCP client.

Endpoint: https://mcp.keepmd.org/eob (Streamable HTTP, no authentication) Documentation: https://keepmd.org/mcp-eob-decoder/ Publisher: KeepMD (VertexMD, Inc.) · [email protected]

In plain English: you paste or describe an EOB or bill to Claude, ChatGPT or another assistant, and the assistant uses this server to say what each code means, what the insurance words mean, whether the amounts add up, and what to do next. The server only ever sees codes, words and dollar figures.

What it does

ToolPurpose
lookup_codesOfficial descriptors for ICD-10-CM diagnosis codes, HCPCS Level II service/supply/drug codes and HCPCS modifiers, with chapter, plain-language notes and licensing notices for CPT, dental, NDC and X12 remark codes (recognized, not described). Up to 50 codes per call.
search_codesFind ICD-10-CM or HCPCS Level II codes from words in a description, or by code prefix.
explain_insurance_term~115 insurance and billing terms (allowed amount, coinsurance, CO/PR group codes, No Surprises Act, observation status…) with examples and related terms.
check_eob_mathRecomputes one service line or claim total from the printed amounts and flags balance billing, mismatched totals and impossible cost sharing.
get_eob_reading_guideStep-by-step guide to reading an EOB, disputing a bill, or appealing a denial, with links to official consumer resources.

Plus the decode_eob prompt and two resources (keepmd://mcp-eob-decoder/glossary, keepmd://mcp-eob-decoder/manifest).

Every tool carries readOnlyHint: true and openWorldHint: false. Every clinical or financial payload carries a disclaimer field: general billing and coding reference, not medical, legal or financial advice.

Privacy

The server has no storage and no accounts. Tools accept codes, terms and dollar amounts only; no schema takes a name, date of birth, member ID, claim number or statement text. Tool arguments are never logged. Full policy: https://keepmd.org/privacy/mcp-eob-decoder/.

Or add the local server to Claude Code with claude mcp add --transport http eob-local http://127.0.0.1:8787/eob and ask it the reviewer prompts in docs/SUBMISSIONS.md. Problems: [email protected] (the issue tracker is off).

Built on @modelcontextprotocol/server (createMcpHandler, stateless Streamable HTTP) and Cloudflare Workers with Static Assets for the code sets. lookup_codes

Look up EOB codes

Checks codes against the current official releases, which change every year, and reports whether each is valid, billable or terminated today. Returns the official descriptor for ICD-10-CM diagnosis codes, HCPCS Level II service/supply/drug codes and HCPCS modifiers found on an Explanation of Benefits or medical bill, plus the chapter, a plain-language note where KeepMD has one, the data version, and licensing notices for CPT, dental, NDC and X12 remark codes (which are recognized but not described). Use it for any code a user asks about, including common ones such as Z00.00, 99213, J0696 or CO-45. Accepts every code from a statement in one call.

Parameterscodesrequiredsystem

search_codes

Search diagnosis and service codes

Finds ICD-10-CM diagnosis codes and HCPCS Level II service, supply and drug codes whose official description matches words from a statement (everyday wording such as "flu shot" or "x-ray" is translated to the clinical terms), or whose code starts with the given characters. Use it when the statement prints a description but no code, or the code is partly illegible. CPT (5-digit) physician-service codes such as office visits and lab panels are not searchable because their text is licensed.

Parameterslimitqueryrequiredsystem

explain_insurance_term

Explain an insurance term

Returns KeepMD's plain-language definition, a worked example and related terms for an insurance or medical-billing term from its glossary (allowed amount, deductible, coinsurance, balance billing, CO/PR group codes, No Surprises Act, observation status and about 110 more). Use it when a user asks what a word, column or abbreviation on an EOB, Medicare Summary Notice or bill means, including common ones such as allowed amount, billed amount, deductible or CO. Takes one term per call.

Parameterstermrequired

check_eob_math

Check EOB arithmetic

Recomputes one EOB line or claim total from the printed amounts (billed, allowed, plan paid, deductible, copay, coinsurance, non-covered, patient responsibility) and reports the contractual adjustment, the expected patient responsibility and plan payment, and any discrepancy such as a balance bill or a mismatched total. Use it when a user gives EOB or bill amounts and asks whether what they owe is right; it returns an itemized, exact calculation with stated assumptions. Pure arithmetic; takes dollar amounts only, never names or identifiers.

get_eob_reading_guide

EOB reading guide

Returns KeepMD's step-by-step guide, red flags and links to official consumer resources for reading an Explanation of Benefits (overview), resolving a bill that looks wrong (dispute), appealing a denial (appeal), or the handful of terms that drive the arithmetic (glossary). Use it when a user asks how to read an EOB, what to do about a bill that looks wrong, or how to appeal a denial.
